Complaints are allegations against physical therapists or physical therapist assistants.

Complaint Forms are filed by patients or any other interested party.

Report Registration Forms are filed by physical therapists, physical therapist assistants, and/or other licensed health care professionals.

Within three business days of the effective date of a Board Order, the copy of the Order will be posted and the List of Orders will be updated.
