Licensee Search

Search for licensees (P.T.'s or P.T.A.'s) by last name or license number.

To include a first name in the search field include the entire last name, add a comma (, ) and space and at least one letter of the first name. For example, to search for licensees with a last name of 'Anderson' and first name starting with 'J' enter: Anderson, J

Note: P.T.A. license numbers start with the letter A. For example:
License number A1000 is a P.T.A and 1000 is a P.T.
